About the Role
The Case Manager Supervisor provides direct casework services to clients/residents and oversees Case Managers.
Responsibilities
- Arranges meetings with newly assigned clients to assess basic needs and develop a service plan.
- Conducts a psychosocial assessment to determine the individual needs of the client.
- Carries an assigned caseload of residents/clients.
- Performs all concrete casework requirements and referrals.
- Completes all required paperwork, case management notes, referral sheets, and statistical requirements on a timely basis.
- Resolves client incident reports and complaint reports as required.
- Participates in all required consumer hearings and discharges.
- Tracks client progress, makes periodic reports of each case, and analyzes results of service plans.
- Develops and implements a treatment plan; tracks progress and evaluates outcomes.
- Makes referrals to appropriate resources to address individual needs; determines eligibility/entitlement for referrals or information.
- Participates in weekly supervision meetings for case and administrative consultation.
- Handles emergencies as they arise; arranging for medical care or other services.
- Attends agency or professional group seminars for on-going development.
- Attends staff meetings as requested.
- Escorts clients to viewings or appointments as needed.
- Updates critical files.
- Updates supervisor notes in all case files and case manager files.
- Completes audit charts for all case files.
- Adheres to all policies and procedures.
- Maintains confidentiality of information learned through the course of the job.
Requirements
- Bachelor’s Degree in Social work or related field
- Certification in First Aid, CPR
- Proficient in computer software programs (Word, Excel, Power Point, CARES, etc.)
- Excellent verbal and written communication skills.
- Ability to read and comprehend simple instructions, short correspondence, and memos.
- Ability to write simple correspondence.
- Ability to present information in one-on-one and small groups situations outside stakeholders, clients and other employees.
- Ability to climb and ambulate stairs (about 10 flights).
- Ability to stoop, kneel, crouch, reach, walk, stand, and lift.
- Ability to use computer tablet to write reports, notes and document compliance.
- Ability to use fingers, hands, arms, and grasp.
- Ability to talk and hear.
- Ability to feel attributes of objects.
- Close visual acuity required for data analysis, computer terminal use, extensive reading, and accuracy checks.
- Must be able to perform the essential functions of the job.
