About the Role

Provides direct service to individuals transitioning back into the community from prison. Services include comprehensive assessment, service planning, coordination of stabilization needs, recordkeeping, case management, collaboration, and outreach.

Responsibilities

  • Demonstrates agency core values in day-to-day activities.
  • Provides comprehensive case management, coordination, and assessment to assigned caseload.
  • Works effectively with parole and other community partners.
  • Works effectively with individuals enrolled to meet benchmarks regarding enrollment, CBI enrollment, and completion.
  • Assures all treatment planning, including convening regular screening, linking, planning, and case conferences involving all providers and support network.
  • Provides outreach to clients, family, and other involved persons.
  • Assures and maintains linkages to medical, financial, social, rehabilitative, addiction, and psychiatric services.
  • Provides effective advocacy to obtain services required by individual clients and to obtain needed systems development or change.
  • Serves as a social change agent by identifying service barriers, participating in the development of solutions, and serving as a systems advocate.
  • Provides required services, including crisis intervention, treatment, and support in the absence of viable alternatives.
  • Performs recordkeeping, statistical tracking, and quality assurance activities accurately and timely.
  • Other duties as assigned.

Requirements

  • Must possess a valid Driver’s License with a satisfactory driving record.
  • Must possess a personal vehicle for job requirements.

Skills

  • Demonstrated ability to work independently and initiate change.
  • Excellent skill and judgment in a range of clinical and case management activities.
  • Ability to collaborate effectively with a team.
  • Superior interpersonal skills in dealing with other providers and client supports.
  • Excellent written and verbal skills.
  • Proficient in the use of computer software such as MS Word, Excel, PowerPoint, Desktop Publishing, Outlook, etc.
  • Excellent time management skills.

Experience Level

  • At least one year of Case Management experience with a bachelor’s degree
  • Two years’ experience with an associate degree
  • Preferred two years’ experience working with individuals involved in the criminal justice system.

Salary/Compensations

  • $23.00-$29.00
