About the Role

Provide support and clinical supervision to case managers and other social service staff, aiming for excellent service provision and helping clients remain stably housed while optimizing their health and well-being.

Responsibilities

  • Ensure provision of quality and timely services, including psychiatric and medical care, entitlements, assessment & treatment planning, and linkages to day programs, clubhouses, employment, and recovery.
  • Act as Liaison with on-site medical and psychiatric team.
  • Coordinate and manage day to day program operations along with Program Director.
  • Supervise case management staff.
  • Ensure all documentation is completed by direct service staff in a timely manner and within established deadlines.
  • Support evidence-based practice models including person centered planning, motivational interviewing, trauma informed care, harm reduction, and strengths-based approach.
  • Ensure collection, tracking, and analysis of data, with a results-oriented mindset.
  • Provide weekly supervision to staff; promote staff development, evaluate their work and initiate disciplinary action when required.
  • Participate in new client interviews, intakes and admission meetings.
  • Coordinate client care with all onsite treatment providers, serve as liaison with medical and psychiatric staff.
  • Assist with audit preparation and performance improvement plans as needed.
  • Assist Program Director with daily program operations, serve as senior leader in Program Director’s absence.
  • Interface with various community agencies, funders and stakeholders relevant to our client population.
  • Coordinate the scheduling of all contracted treatment providers and consultants.
  • Collaborate with staff in other GRCC programs to coordinate client service.
  • Work with the Clinical Integration Team on the clinical best practices initiative.
  • Attend staff meetings, case conferences, training programs and supervision meetings.
  • Participate in developing group curriculum and facilitate group activities as needed.
  • Perform other duties as required.

Requirements

  • Supervisory experience required.
  • Experience in supportive housing a strong plus.
  • Experience working with individuals diagnosed with mental illness, substance use and homelessness strongly preferred.
  • Bilingual (English and Spanish) a plus.
  • Able to work in the Microsoft Office Suite, should especially be proficient in Word and Outlook.
  • Able to use or learn to use AWARDS database.
  • Familiar with cloud-based payroll platforms such as Paycom (supervisor/manager functionality).
  • Able to use or learn other electronic databases, programs, and funder-related software as needed.
  • Frequent walking and standing, occasional lifting.
  • Some visits to client rooms required.

  • The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.

  • Bachelor’s Degree considered with significant experience

Salary/Compensations

  • $70,000.00 - $75,000.00

  • Stephen Russo Residence (W 107th Street), is a permanent supportive housing residence for formerly homeless adults with mental health and co-occurring disorders. Services include case management, harm reduction, housing readiness, counseling, medical and psychiatric services, groups and recreation, and 24-hour 7 day a week front desk concierge.
