About the Role

Grant funded position requiring travel to two local shelters in New London. This role supports primary care providers by offering clinical and technical assistance, implementing planned care using electronic tools, monitoring equipment, and performing clinical testing.

Responsibilities

• Applies evidence-based guidelines from major regulatory and specialty groups.
• Performs pre-visit preparation for all primary care patients.
• Collaborates with primary care nurse and provider for 'e-huddles' using the Planned Care Dashboard.
• Prepares patients for provider visits, including telehealth technology guidance.
• Manages virtual telehealth sessions using Zoom technology.
• Supports clinical teams with paperwork, record requests, and scheduling follow-up visits.
• Prepares patients to be seen by provider, using established criteria.
• Accurately documents chief complaint and results from evidence-based screening tests.
• Accurately performs and documents patient vital signs.
• Independently and accurately performs appropriate CLIA waived tests, EKG, hearing & vision tests.
• Prioritizes work responsibilities to decrease waiting time.
• Collaborates with providers, clinical and clerical support staff to facilitate appropriate patient flow.
• Works effectively as a member of the health care team, assisting other team members as needed.
• Appropriately schedules tests and referrals for specialty care in accordance with Provider orders.
• Maintains a clean, orderly, and safe working environment.
• Oversees maintenance of supplies and documents in medical logs.
• Sets up and stocks exam rooms and cleans rooms at the end of each day worked.
• Adheres to all infection control standards, Universal Precautions, and safety policies.
• Maintains yearly CPR certification.
• Exhibits excellent customer service skills with a focus on quality of care and teamwork.
• Complies with Agency mandated in-service training workshops.
• Assists the primary care nurse with provider clinic support such as routine medication administration and immunizations.
• Communicates clinical/client information to providers accurately.
• Accurately takes messages and communicates on the telephone.
• Communicates effectively with other medical assistants, collaborates with medical provider and nursing staff.
• Maintains client confidentiality according to CHC policies.

Requirements

• Must have reliable transportation to travel to two local shelters in New London.
• LPN Program Completion Certificate.
• Current CT LPN License.
• CPR—American Red Cross CPR for the Professional Rescuer and AED certification or American Heart Association BLS/AED for Healthcare Providers.
• High school diploma or equivalency.
• Completion of a Medical Assistant course from an accredited/recognized school.
• Medical Assistant board certification (AAMA, NHA)/registration (AMT) or achieved within 90 days of hire.
• CHC requires as a condition of employment current American Red Cross CPR for the Professional Rescuer and AED (CPR/FPR/AED) certification. The only acceptable alternative is current American Heart Association BLS/AED for Healthcare Providers certification.
