About the Role

The Community Habilitation Specialist provides support in a home or community setting that promotes success at the highest level of independence possible. This role involves teaching and reinforcing daily living skills, promoting independence, preventing and responding to crisis situations, encouraging self-advocacy, and facilitating the development of natural supports, all within a person-centered approach.

Responsibilities

  • Establish respect and develop relationships with individuals using a person-centered approach.
  • Become familiar with an individual through assessment, observation, and discovery.
  • Provide evaluation of the person’s skills and capacity for skill development in their home and community.
  • Provide teaching and assistance for daily living tasks such as cleaning, laundry, cooking, personal care, and banking.
  • Promote advocacy by seeking out services, promoting choice, and assisting in understanding rights and responsibilities.
  • Assist the individual with communication with their support team, including families, natural supports, and community resources.
  • Respond to crisis situations by using good judgment through prevention, intervention, resolution, and reporting.
  • Maintain necessary records and documentation for each consumer served.
  • Submit monthly notes for review by the Director of Programs.
  • Meet at least bi-weekly with the direct supervisor to provide updates and discuss progress and concerns.
  • Report all incidents to the Director of Programs and follow incident reporting procedures.
  • Maintain confidentiality of all work-related information.
  • Act in a manner consistent with Central New York Quest’s Mission.
  • Perform other duties as assigned by the Director of Programs.

Requirements

  • Valid Driver's License
  • Willing to transport clients in own vehicle
  • 2 years experience working with individuals with developmental disabilities (preferred)
  • High School Diploma
