About the Role

The Senior Residential Counselor is responsible for assisting the Program Director in all aspects of residential operations, ensuring compliance with regulatory requirements and PCMH policies. Key responsibilities include quality case management, maintaining records, supervising staff, and crisis response.

Responsibilities

  • Ensures residents follow psychiatric and medical plans and approves individual service plans.
  • Integrates Health Care Coordinator services with other program and case management services.
  • Maintains case records in accordance with regulatory requirements and reviews individual case records.
  • Provides counseling to residents as required.
  • Maintains a resident caseload as needed.
  • Assists in scheduling and planning recreational and social activities.
  • Facilitates groups in the residence.
  • Manages crises within the residence and intervenes appropriately.
  • Notifies supervisor and documents all untoward incidents and completes follow-up.
  • Orders, supervises, and counts medication according to policy.
  • Prepares written correspondence and notifications on resident-related issues.
  • Escorts clients in the community to appointments.
  • Maintains facility safety and security, monitors surveillance cameras, and conducts building walk-throughs.
  • Documents significant building activities and events in the Communication Logbook.
  • Performs light maintenance and housekeeping.
  • Conducts monthly fire drills.
  • Conducts apartment inspections and supports clients with daily living skills.
  • Participates in residential staff meetings.
  • Remains on site until relief staff arrive.
  • Provides ongoing supervision and directives to Residential Counselor and Front Desk Counselors.
  • Conducts internal audits of clinical case records and program documentation for compliance.
  • Monitors compliance with safety and health code requirements.
  • Maintains productive working relationships with provider groups and referral sources.
  • Adheres to personnel and residential policies and procedures.
  • Participates in special projects assigned by senior management.
  • Performs other duties as assigned by the Program Director or agency administration.

Requirements

  • Minimum of a Bachelor’s Degree in a Mental Health discipline or related field with 3 years related experience preferred.
  • High School Diploma with at least 5 years of related experience in a residential setting considered.
  • Computer literate with knowledge of Microsoft Office, spreadsheets, and publishing software.
