About the Role
The Client Care Coordinator is responsible for the overall delivery and coordination of mental health and related services for homeless individuals. This role will enhance existing services by assessing clients' long-term goals and collaborating with case management to set achievable goals and establish community linkages for successful progress towards independent living.
Responsibilities
- Complete a comprehensive bio-psychosocial assessment with each client to understand strengths and service needs.
- Provide clear written reports detailing assessment findings and recommendations.
- Assist homeless individuals in navigating multiple systems and coping with the stressors of homelessness.
- Provide outreach services, crisis intervention, risk assessment, safety planning, and psycho-education.
- Prioritize and conduct unit visits to high-risk individuals to coordinate safety planning.
- Prepare individuals to accept services.
- Collaborate with ACS and/or prevention services agencies and participate in conferences to advocate on behalf of clients.
- Make appropriate referrals and facilitate linkages between mental health providers, government agencies, and community-based services.
- Serve as an agency/program liaison between government agencies, community-based organizations, and groups.
- Improve access to mental health services by facilitating in-person or telehealth services.
- Enhance the delivery and coordination of mental health and related services.
- Work effectively as part of a multi-disciplinary team to enhance engagement and ensure quality service.
- Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being.
- Deliver enhanced mental health services, including biopsychosocial assessments, on-site mental health services, clinical care coordination, referral, linkage to care, and follow-up.
- Promote in-person or telehealth usage by utilizing available service utilization platforms.
- Utilize DHS Referral Basics to match needs, assist clients with scheduling, provide reminders, coordinate transportation, facilitate teletherapy access, and provide warm hand-offs.
- Follow up on referrals to confirm attendance, session outcomes, and address concerns; assist with rescheduling and barrier resolution if a client did not attend.
- Utilize motivational interviewing to address client reluctance, misconceptions, remove barriers, and re-engage clients.
- Provide therapeutic groups (music, art, general support groups for caregivers or children) to address family issues and enhance life skills.
- Participate in family meetings with case management and housing staff to discuss planning efforts and progress.
- Assist clients with completing applications for supportive housing and accompany residents to interviews.
- Upon linkage to permanent housing, complete and discuss exit summaries with clients, and provide referrals and supportive services for their new community.
- Utilize fact sheets, desk guides, and other material resources in delivering mental health services.
- Complete all documentation in a timely manner, documenting services provided, efforts, and outcomes in CARES.
- Compile information/data for internal and external reports.
- Attend professional development training and opportunities; participate in mental health trainings.
- Perform other duties as assigned.
Requirements
- Must be a Licensed Master Social Worker (LMSW) in the state of New York upon hire or obtain licensure within three (3) months of hire.
- Master’s degree in social work from an accredited school of social work.
- At least one year of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services is strongly preferred.
- Knowledge and experience working with diverse cultures and ethnicities.
- Expertise in strengths-based, solution-focused, and family-centered practice.
- Experience facilitating groups for adults.
- Excellent written and verbal communication skills.
- Excellent interpersonal skills.
- Computer literacy in Microsoft-Office is required.
- Lifting and moving objects weighing up to 30 pounds several times a week.
