About the Role
Health Link Home Health and Hospice is seeking a compassionate, organized, and team-oriented Full-Time Home Health LVN to serve patients throughout San Francisco. This role involves collaborating closely with the Director of Nursing, clinical staff, and office team to ensure coordinated, high-quality care, supported by a dedicated back-office team and modern tools like AI charting software.
Responsibilities
- Implement the plan of care initiated by the Registered Nurse
- Provide accurate and timely documentation consistent with the plan of care
- Assess and educate patients and caregivers regarding diagnosis, care plans, and treatments
- Participate in coordination of home health services, reporting additional needs (HHA, PT, OT, ST, MSW) to the RN or Clinical Supervisor
- Use equipment and supplies effectively and responsibly
- Complete online charting in Kinnser using company-provided devices
- Follow all company policies and procedures
- Perform other duties as assigned
Requirements
- Minimum 1 year of nursing experience in a medical setting
- IV certification and experience with wound care
- Home health experience recommended but not required
- Active California LVN license
- Valid California Driver’s License
