About the Role

Provide Speech-Language Pathology Therapy and related services for patients in an Assisted Living, Independent Living, and Memory Care setting. This role involves screening, evaluation, treatment planning, and patient care while maintaining positive interactions with facilities and clients.

Responsibilities

  • Optimize communication, cognitive-linguistic abilities, and swallowing skills of patients through speech therapy.
  • Screen residents and evaluate/treat patients referred by state-approved sources.
  • Ensure accurate and timely billing and documentation in compliance with state and federal guidelines.
  • Establish and maintain effective, patient-centered interactions with patients, the interdisciplinary team, and other clientele.
  • Directly supervise Rehab techs, promoting efficient resource utilization and clinical skill development.
  • Actively integrate SLP therapy's role into the community and interdisciplinary treatment team.
  • Contribute to community patient care, utilization review, case management, and family conference activities.
  • Establish and maintain good working relationships with other healthcare professionals and colleagues.
  • Make appropriate referrals to other members of the rehabilitation team.
  • Develop and implement new programs and techniques to enhance rehabilitation services.
  • Adhere to Reliant Rehabilitation and client-specific policies regarding safety, risk management, infection control, and emergency preparedness.
  • Adhere to patient rights and privacy requirements.
  • Enhance clinical expertise and professional skills through continuing education and self-study.
  • Maintain awareness of current issues in speech-language pathology therapy and the healthcare environment.
  • Understand various payment models, billing and treatment guidelines, and clinically appropriate means of delivery.
  • Provide quality care with reasonable goals and outcomes.
  • Comply with infection control procedures and environmental safety protocols.
  • Perform patient lifting using appropriate biomechanical techniques, capable of lifting thirty (30) pounds of dead weight alone.
  • Maintain confidentiality of Reliant Rehabilitation, community, and patient information.
  • Conduct self in an ethical, legal, and responsible manner at all times.
  • Adhere to the policies, principles, and guidance within the Team Member Handbook and Code of Business Conduct.
  • Travel between facilities during scheduled hours.
  • Work varying hours and shifts, including weekends and holidays.
  • Attend all mandatory meetings, trainings, and assignments.
  • Perform other duties and responsibilities as assigned.

Requirements

  • Master’s Degree, or equivalent, in Speech Language Pathology.
  • Proficient knowledge of Microsoft Office (Outlook, Word, Excel, Power Point, etc.).
  • Basic Internet navigation skills.
  • Valid Speech-Language Pathologist state license (where appropriate).
  • Effective clinical problem-solving skills.
  • Ability to work in a fast-paced, productive work environment.
  • Ability to build strong relationships with a diverse population.
  • Ability to read, write, speak, and comprehend in English: instructions, correspondence, charts, memos, and reports.
  • Must be capable of lifting thirty (30) pounds of dead weight alone.
