About the Role

The Licensed Practical Nurse is responsible for the management of the client medication system and monitoring the general health of clients.

Responsibilities

• Conduct health screens for all new clients during the shelter intake process.
• Utilize motivational interviewing, recovery, and trauma-informed approaches when delivering services to clients.
• Administer medication and document appropriately in the Medical Administration Record (MAR).
• Ensure accuracy of documentation in MAR.
• Inform providers of necessary medication refills for clients in a timely manner.
• Inform the pharmacy and receive prescriptions for medication refills.
• Remove expired or redundant medications from the medication cart.
• Respond to emergencies and ensure follow-up with emergency services.
• Collaborate with the PNP in coordinating follow-up care.
• Educate clients on medication and treatment compliance strategies.
• Communicate all client medical concerns to the PNP and assist the PNP in determining medical care needs and coordination of care.
• Monitor vital signs and clients’ response to medications and side effects.
• Review findings with the PNP and assist with follow-up medical care as needed.
• Order medical supplies as needed and store supplies and medication records in a secured closet.
• Secure the medication cart in a locked, secure location.
• Participate in team meetings, case conferences, staff meetings, and trainings.
• Collaborate with the social service team in assessing clients’ needs, developing service plans, and coordinating care for clients.
• Document delivery of services, client progress, etc., in the electronic case record in accordance with agency policies and procedures.
• Contribute to the overall success of the organization by performing all other duties and responsibilities as assigned by supervisor or designee.
• Be able to pivot with ever-changing priorities and newly added job-related responsibilities that arise based on business changes, needs, and demands.

Requirements

• NYS Licensed Practical Nurse is required.
• Current First Aide/CPR Certification.
• Minimum two (2) years of post-licensure experience providing nursing care in a clinic setting or social service setting is required.
• Excellent analytical, evaluative, and human service management skills.
• Ability to think and act calmly in emergency situations.
• Attention to detail and problem-solving skills.
• Strong organizational skills with the ability to manage tasks efficiently.
• Superb time management skills with the ability to prioritize assignments.
• Exceptional written and oral communication, organizational, supervisory, and leadership skills.
• Must have incredible initiative, the ability to work well under pressure, and the ability to be flexible and work both independently and as part of a team to complete complex projects in a fast-paced environment.
• Capable of developing strong teams who can develop collaborative working relationships and function within the interdisciplinary team.
• Must be adept and comfortable with crisis management.
• Ability to work effectively with a wide range of community-based providers.
• Familiarity with DHS and OTDA social service documentation policies and procedures.
• Critical thinking, analytical and problem-solving skills as well as resourcefulness.
• Advanced level of computer proficiency with proficiency in Word, Excel, and the ability to learn new technologies and databases.
• May be required to work long hours and a varied schedule.
• Traveling throughout various locations in the NYC area.
• Lifting and moving objects weighing up to 30 pounds several times per week.
