About the Role
Leland is seeking its first Patient Advocate to serve as the primary point of contact for families, managing their end-to-end care journey and developing the training playbook for future advocates. This role involves owning family relationships, coordinating care, leveraging AI tools, verifying information, and documenting services for billing.
Responsibilities
- Own the family relationship from beginning to end.
- Manage all communications, referrals, and follow-ups after discharges, referrals, and medication changes.
- Accept, edit, or reject AI-suggested next best actions.
- Utilize AI agents for tasks like booking, paperwork, and managing holds.
- Personally verify all information provided to families.
- Escalate issues requiring new clinical judgment to physicians.
- Document all actions, communications, and time accurately for billing purposes.
- Develop the advocate playbook, including scripts, checklists, and escalation rules, based on initial service experiences.
Requirements
- Current, unrestricted licensure in good standing as an RN, NP, or PA.
- 3+ years of patient-facing experience with older adults in roles such as navigation, care coordination, nursing, or case management.
- Must be based in New York City.
- Demonstrated ability for fierce follow-through and ownership.
