About the Role

Responsibilities
- Demonstrate accountability for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients.
- Act as a health coach, helping patients set and achieve health goals through frequent follow-up.
- Simplify care processes to make behavior change actionable for patients and guide care teams.
- Manage patient admissions, appointment slots, patient experience, clinical gap closures, and medical cost measures.
- Champion a positive culture of love, accountability, and passion within the medical center.
- Function independently and collaboratively with other healthcare professionals.
- Ensure compliance with guidelines and participate in risk and quality management programs.
- Adhere to departmental goals, standards of performance, regulatory compliance, quality patient care, and policies and procedures.
- Provide independent care for patients with acute and chronic illnesses in older adults.
- Take full accountability for patient care and outcomes, seeking specialist consultation when needed.
- Engage with hospitalists when patients are hospitalized.
- Perform assessment, diagnosis, treatment, management, education, health promotion, and care coordination for patients with acute and complex chronic health needs.
- Lead care teams consisting of care promoters, facilitators, and coordinators for in-office patients.
- Engage with transitional care teams and other resources for patients unable to come to the office.
- Actively participate in center management and cover for other providers as needed.
- Take an active role in recruiting patients for the center and additional providers for the company.
- Supervise, collaborate with, or function within a practice agreement with an Advanced Practice Practitioner (APP), remaining accountable for their actions.
- Perform other duties as assigned.
Requirements
- Must be able to obtain a State Medical License or already have a current, active State Medical License.
- Must have a current DEA number for schedule II-V controlled substances.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within the first 90 days of employment.
- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook.
- Ability and willingness to travel locally as needed, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training purposes.
- Fluency in English, verbal and written.
- Requires use and exercise of independent judgment.
- Flexible to work evenings, weekends, and/or holidays as needed.
