About the Role

The Licensed Practical Nurse, under nursing supervision and direction of an RN, implements the nursing plan. Responsibilities

  • Provides nursing care under the supervision and direction of an RN.
  • Administers medications and immunizations.
  • Observes, measures, records, and reports clinical data relating to patient’s health status.
  • Performs clinical procedures such as catheterizations, trach suctioning, and dressing changes.
  • Provides emotional support and health teaching to patients and families.
  • Communicates with primary CHN/Home Health RN and other professional disciplines regarding tolerance, progress, compliance, and deviations from the plan of care.
  • Maintains current, complete, and accurate clinical records per Schofield Certified Home Care policy and regulatory guidelines.
  • Delivers patient care, treatments, and medication administration to assigned patients.
  • Oversees HHAs and PCAs.
  • Performs other related duties in accordance with SCHC policies, as required by their manager.

Requirements

  • Graduate of a NYS approved LPN program.
  • Current New York State Licensed Practical Nurse Registration.
  • Two (2) years experience in a Home Health Agency (CHHA, LTHHCP, or Hospice) and/or Long Term Care preferred.
  • One (1) year previous medical/surgical experience within the last five years.
  • Must possess good moral character, patience, and compassion for the elderly, chronically ill, and disabled.

  • Fulltime Monday through Friday 8:30am - 4:30pm

Experience Level

  • Two (2) years experience in a Home Health Agency (CHHA, LTHHCP, or Hospice) and/or Long Term Care preferred.
  • One (1) year previous medical/surgical experience within the last five years.

Education Level

  • Graduate of a NYS approved LPN program.
  • Current New York State Licensed Practical Nurse Registration.

Salary/Compensations

  • $26.50 - $33.00 Hourly
