Responsibilities
- Secure Medicaid and other relevant benefits.
- Interview and screen individuals for needs and program eligibility.
- Assist individuals in maximizing community integration and normalization.
- Provide leadership in coordinating resources.
- Refer individuals to community and social support services.
- Provide transitional care and linkage, including follow-up from inpatient and other settings.
- Support individuals in navigating complex healthcare and social service systems.
- Offer support to individuals and families toward achieving their goals.
- Advocate for needed services.
- Provide on-site crisis intervention and skill teaching when other services are unavailable.
- Monitor and follow up on contacts and activities to ensure services align with treatment plans and address individual needs and wellness recovery goals.
- Monitor progress by collaborating with the individual, their family, service providers, and other relevant entities.
- Consult with collaterals, with client permission, to improve individual outcomes and assist with goals.
- Take a lead role in collaborating with service providers for continuity of care.
- Prepare and maintain consumer records according to departmental procedures.
- Develop services to meet client needs when no appropriate services exist or are inadequate.
- Report emergencies and concerns to supervisor/manager/director and work toward resolution.
- Perform other duties as assigned to meet consumer and department needs.
- Adhere to agency policies and procedures.
Requirements
- High school diploma or equivalent.
- Work with mental health or substance use populations preferred.
