About the Role

A Psychosocial Rehabilitation Specialist serves in a paraprofessional capacity as part of the crisis intervention unit. This position assists clients in developing and implementing their personal wellness goals, supporting recovery-oriented activities, interventions, and skill development.

Responsibilities

• Assists clients with paperwork related to pending admissions, discharges, and post-discharge phases.
• Provides transportation for patients/clients to and from inpatient psychiatric and crisis stabilization placements.
• Performs a backup role to the crisis worker with support and informational crisis phone contacts.
• Assists with the coordination of care for clients in crisis stabilization sites.
• Provides in-person medication deliveries, welfare checks, and phone prompts to clients.
• Assists individuals in identifying meaningful life role goals and objectives through a person-centered process.
• Trains individuals in coping skills to address symptoms, manage stress, and reduce vulnerability to stress.
• Assists in identifying trauma triggers and developing healthy coping alternatives.
• Encourages personal autonomy through stress management skills and recognition/coping with mental health symptoms.
• Encourages development of ways to manage free time.
• Assists in navigating transportation.
• Guides individuals in understanding and managing chronic medical conditions.
• Engages in further development of health social skills.
• Educates individuals in wellness activities.
• Teaches and strengthens daily living skills.
• Encourages development of financial management strategies.
• Assists in pursuing, securing, and retaining integrated, competitive employment.
• Assesses ongoing progress toward recovery and functional skill development.
• Maintains all required documentation in a timely manner.
• Communicates and reports to Supervisor.
• Actively participates in timely completion of assigned trainings.
• Manages a caseload of clients to establish and implement individual health and wellness plans.
• Assists in establishing a wellness support system outside of the PRS.
• Consults with collaterals, with client permission, to improve client outcomes and assist them in addressing their goals (for children).
• Provides psychoeducation to restore and support community tenure and avoid more restrictive placements (for children).
• Enhances personal relationships, establishes support networks, and increases community awareness (for children).
• Develops coping strategies toward effective functioning in all relevant environments (for children).
• Teaches and strengthens daily living skills to assist with self-management and reduction of challenges (for children).
• Assists in developing wellness skills (for children).
• Supports personal autonomy skill development (for children).
• Enriches personal skills (for children).
• Develops health skills (for children).
• Identifies personal interests and hobbies (for children).

Requirements

• Must have a clean, valid driver’s license.
• High school diploma or equivalent and at least 1 year of experience working with individuals with substance use disorders and/or serious mental illness OR bachelor’s degree and 1 year relevant experience (for Adults).
• High school diploma or equivalent and at least 3 years of relevant experience working with children involved with mental health, addiction and/or foster care (for Children).
