About the Role
The Supported Housing Coordinator provides leadership, guidance, and support to the Supported Housing team, ensuring individuals receiving services have the necessary tools and resources to maintain housing, achieve their goals, and live successfully in the community. This role is crucial for the day-to-day coordination of the program, encompassing staff supervision, training, quality assurance, documentation, program development, and collaboration with community partners. The Coordinator will also manage a smaller caseload to stay connected with service recipients while supporting the team.
Responsibilities
- Supervise Supported Housing staff, provide coaching and guidance, and conduct performance evaluations.
- Provide training, support, and feedback regarding best practices, agency policies, regulatory requirements, and quality service delivery.
- Facilitate staff meetings, encourage open communication, and foster a positive team environment.
- Monitor program goals, outcomes, documentation, and reports to ensure timely and effective services.
- Conduct Quality Assurance reviews of staff documentation for accuracy and completeness in the electronic health record.
- Maintain a caseload and provide direct support to recipients, assisting with goals, housing stability, recovery, and community integration.
- Represent the agency at community meetings, including SPOA and other service coordination forums.
- Assist staff and recipients with challenges, conflicts, crises, and barriers impacting housing stability or community living.
- Collaborate with community providers, treatment teams, families, and other service professionals.
- Assist with program development, monitoring, evaluation, and identifying service improvement opportunities.
- Participate in off-hours on-call and emergency response responsibilities.
- Work occasional evenings and weekends as needed.
- Participate in agency, program, and community meetings.
- Assist with special projects, program initiatives, and program development activities.
- Provide additional support to the Supported Housing team during periods of increased need.
- Identify creative solutions to improve services and strengthen the program.
- Take on additional assignments as needed.

- Able to work in open space floor plan.
- Must be capable to move throughout work day.
- Work alongside co-workers within 3 feet.
- Must be able to move in tight spaces.
- Occasional lifting of > 15+ pounds.

Experience Level
- Minimum of two (2) years in the provision of mental health services, including promoting recovery, providing crisis management, and coordinating and collaborating with community resources.
- Supervisory experience preferred.
- Background working with individuals with mental health needs in a community-based setting preferred.
- Familiarity with electronic health records and service documentation preferred.

- Bachelor’s degree
