About the Role

Case Managers empower clients to achieve their goals by creating a collaborative and supportive working relationship based on empathy, accountability, and trust. They provide information, referrals, and coordinate services such as health care, job training, housing, and immigration. Case Managers develop strategies for client engagement, community service awareness, and re-housing.

Responsibilities

  • Carry a caseload of 25 clients.
  • Ensure all assigned clients are interviewed and assessed within 24 hours of admission.
  • Conduct intakes and assess residents’ needs, coordinating referrals.
  • Develop individualized service plans with short- and long-term goals in collaboration with residents, reviewing plans bi-weekly.
  • Monitor client compliance with goals through weekly scheduled appointments.
  • Maintain orderly and up-to-date client files, documenting and addressing missed appointments.
  • Work closely with shelter staff to enforce client rules and regulations.
  • Hold conferences with clients when rules are broken.
  • Serve as an advocate with agencies and service providers to minimize barriers to client services.
  • Follow up with referral sources to ensure clients receive necessary services promptly.
  • Accompany clients to appointments when necessary.
  • Conduct apartment inspections to expedite re-housing.
  • Coach clients in job preparation, housing availability, social services, and other areas.
  • Represent CP Unlimited at city and statewide coalitions, task forces, and committees.
  • Participate in trainings, meetings, case conferences, and training seminars.
  • Comply with trainings required by the Department of Homelessness or Constructive Partnership Unlimited.
  • Perform other duties as assigned by the Case Work Supervisor.

Requirements

  • Bachelor’s degree in human services (no experience required) or a bachelor's degree in a field outside of Human Services with one year of experience in a Human services program or congregate setting.
  • Demonstrate sensitivity and experience with and/or knowledge of disenfranchised communities including individuals who identify as lesbian, gay, bisexual, transgender, queer, and/or HIV-affected, and individuals with mental health issues and homelessness.
  • Experience with the homeless population preferred.
  • Good written and oral communication skills.
  • Ability to work under pressure and respond to crises, health, or safety issues with appropriate and immediate action.
  • Ability to work flexible hours, including evenings and weekends.
  • Computer-literate with knowledge of Microsoft Office applications.
  • CPR/First Aid certification required.
  • Narcan Certification required.
  • Bilingual in Spanish, French, and/or French Creole preferred.
