About the Role
We are looking for a compassionate, organized, and team-oriented Occupational Therapist (OT) to join our team. You will provide skilled occupational therapy services focused on ADLs, adaptive equipment, and functional independence within a dedicated territory.
Responsibilities
- Provide skilled occupational therapy services focused on ADLs, adaptive equipment, and functional independence
- Develop, implement, and manage individualized plans of care based on physician orders
- Report changes in patient condition and collaborate with physicians, case managers, and the Director of Nursing
- Maintain accurate, timely clinical records using AI-supported documentation tools
Requirements
- Current, active California Occupational Therapist license
- Minimum of 1 year of home health experience strongly preferred
- Valid California driver's license

Salary/Compensations
- $125,000 – $150,000 per year (based on experience)
- Total Earning Potential: $156,250 – $187,500 per year
