About the Role
Professional Care Medical Practice P.C. is seeking a Licensed Mental Health Counselor (LMHC) or Licensed Social Worker (LMSW or LCSW) to serve as a Behavioral Health Professional within the medical department at the Monroe County Jail in Rochester, NY.
Responsibilities
- Conduct clinical assessments, including documentation, case conceptualization, diagnosis, and treatment plans.
- Perform risk assessments using structured interviews and tools, managing inmates at risk for suicide or self-harm according to protocols.
- Serve as the mental health liaison for services outside the facility.
- Facilitate admissions and placements to forensic hospitals when necessary.
- Monitor all mental health services provided.
- Ensure clinical services meet NCCHC and accepted psychiatric/psychological standards.
- Participate as a member of the Quality Assurance Committee.
- Develop and conduct in-service education programs.
- Collect data and prepare monthly performance reports.
Requirements
- Must be licensed as a LMHC, LMSW, or LCSW in New York State.
- Must hold a Master’s degree or greater in Social Work, Psychology, or a related Human Service field with a concentration in clinical coursework, training, and/or experience.
- Must hold a valid driver’s license.
- Must have reliable transportation.

- $65,000.00 - $75,000.00
