About the Role

As a Medication Coordinator, you will administer medication to clients, ensure prescriptions are documented, and maintain an orderly system. You will collaborate with the client service team to achieve program goals, ensuring client safety and well-being.

Responsibilities

• Administer medication to clients as needed.
• Ensure all prescriptions are documented and maintained in an orderly fashion.
• Work cooperatively with other team members to carry out program goals and objectives.
• Ensure the safety and wellbeing of clients on a daily basis.
• Assist clients and case managers in filling prescriptions and ordering refills.
• Ensure physician orders are current, properly documented, and communicated to staff.
• Refer clients to emergency care if needed, coordinating with emergency responders and staff.
• Coordinate treatment and discharge planning with hospital staff.
• Report any verified or suspected abuse or neglect in accordance with policy and procedure.
• Maintain HIPPA protocols, confidentiality, and privacy regarding all client information.
• Prepare clients for self-medication status and monitor as needed.
• Conduct health assessment referrals.
• Monitor client medical equipment for safety and proper functioning.

Requirements

• High School Diploma or equivalent OR equivalent experience and skills.
• AMAP Certification.
• CPR/First Aid Certification.
• At least one year of experience in direct clinical work with psychiatrically disabled populations.
• Ability to work independently and as part of a team to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences.
• Computer literacy, particularly with Microsoft Office applications.

Skills

• Bilingual (English/Spanish) preferred.
