About the Role

Girling Health Care of NY is seeking an experienced and reliable home care Occupational Therapist to conduct home visits in Manhattan, NY. The role involves providing therapy services to patients according to a written physician's plan of care, including assessments, evaluations, and participation in the total plan of care.

Responsibilities

  • Assess occupational therapy needs using objective diagnostic and prognostic tests.
  • Prepare initial patient assessments and perform periodic reassessments.
  • Develop a plan of care aligned with the practitioner's prescription, detailing services, duration, and visit frequency.
  • Establish measurable short and long-term goals with timeframes for patient outcomes.
  • Provide occupational therapy treatment in collaboration with the patient's physician.
  • Document patient response to treatment and modify the plan as needed after physician notification.
  • Instruct healthcare personnel, patients, and families on techniques for mobility and function restoration.
  • Complete case conferences, 60-day summaries, transfer, and discharge summaries.
  • Prepare all documentation on appropriate forms.
  • Participate in the development and revision of the physician's Plan of Treatment with the Home Care Coordinator.
  • Attend case conferences, staff meetings, and in-service programs as designated.
  • Collaborate with the Interdisciplinary Team to develop interventions for clinical and functional goals.
  • Participate in the agency's Quality Assurance Performance Improvement/PIP.
  • Submit visit notes weekly or within 7 days of each home visit.
  • Provide written Home Exercise Programs and instruct patients/families/caregivers.
  • Demonstrate sound judgment regarding suspected corporate compliance violations.
  • Report all suspected violations to supervisors or the Compliance Officer.
  • Perform other duties as assigned.

Requirements

  • Graduate of an approved Occupational Therapy school.
  • Licensed and currently registered as an Occupational Therapist in New York State.
  • Two years of institutional and/or home care experience.
  • Satisfy state health status requirements.

  • $100,000.00 - $120,000.00 per year
