About the Role
The Program Supervisor directly oversees and supervises the Care Coordination Team, ensuring Medicaid compliance, managing billing processes, and utilizing health information technology. This role guides the program towards Health Home goals, involving significant supervisory, administrative, and program management responsibilities. The goal is to ensure the team functions optimally, meets contractual obligations, and fosters a client-centered, outcome-oriented culture of continuous quality improvement.
Responsibilities
- Provides clinical and administrative supervision to Care Coordinators.
- Monitors staff performance, provides training, and coaches for improvement.
- Manages staff recruitment and hiring.
- Assesses staff skill levels and develops individualized training plans.
- Recommends internal/external workshops and trainings for staff development.
- Tracks staff work activities including outreach, intake, assessment, service planning and delivery, referrals, follow-up, collaboration, documentation, confidentiality, and contact standards.
- Develops and implements performance improvement plans.
- Manages the progressive disciplinary process as needed.
- Writes and delivers performance appraisals.
- Conducts quality assurance reviews to assess service quality and effectiveness.
- Ensures member charts and documentation comply with Health Home agreements.
- Develops and implements action plans to improve staff effectiveness and efficiency.
- Oversees care provided by the care management team through case conferences, field visits, and customer service calls.
- Responds to inpatient and ER admission alerts and contacts medical providers/hospitals for admission/discharge information.
- Assists in planning and implementing operations procedures.
- Provides program management with continuous feedback about operations.
- Serves as the point of contact for crisis intervention services.
- Maintains 24/7 availability for staff and client emergencies.
- Ensures staff maintain clinical documentation and records that uphold HIPAA regulations.
- Utilizes multiple Electronic Health Record systems.
- Maintains client assignments and roster maintenance.
- Reviews all billable contact notes and ensures monthly billing is submitted by the first business day of the month.
- Responsible for weekly, monthly, and quarterly reporting of program information.
- Develops, delivers, and participates in ongoing professional trainings.
- Represents PCMH in local and state meetings as required.
- Performs other duties as assigned by the Program Director and/or Executive Management.
Requirements
- Masters degree required.
- 4 years of experience working with the seriously mentally ill.
- Experience supervising the provision of services to Medicaid recipients with chronic medical and mental health issues.
- Knowledgeable about Health Home regulations.
- Comfortable working with clients in the field.
