About the Role

Access Alliance Multicultural Health and Community Services (AAMHCS) is seeking a thoughtful, empathic, and organized individual to join their interdisciplinary health services team. The Counsellor/Therapist will provide assessment, case management, counseling, and advocacy for clients facing complex health and social issues, working within a collaborative model of practice.

Responsibilities

• Provide brief and short-term trauma-informed counseling for clients.
• Support newcomers with mental health needs using culturally appropriate, trauma-informed, and emerging practices.
• Coordinate client care, including orientation, assessment, referrals, case management, and discharge planning.
• Participate in the Centre's interdisciplinary team to support integrated client services based on social determinants of health.
• Establish and maintain links with community organizations and groups.
• Work with community groups to identify service gaps and needs.
• Network, liaise, and advocate with organizations and community groups on identified issues.
• Participate in primary health care program and service delivery in partnership with other institutions.
• Participate in organizational systems including quality assurance and performance evaluation.
• Travel throughout the City to designated sites and work flexible hours, including evenings and weekends.

Requirements

• Master's degree in social work and registration or eligibility for registration with the Ontario College of Social Workers and Social Service Workers.
• Minimum 3 years full-time work experience in clinical counseling.
• Thorough familiarity and work experience with therapeutic methods and practices based upon a health promotion model.
• Excellent case management, casework, counseling, crisis intervention, and group facilitation skills.
• Ability to travel within Toronto and work flexible shifts including evenings and Saturdays.
• Strong computer literacy (word processing, internet, data entry in Windows applications, electronic medical record charting).
• Extensive knowledge of issues related to working with refugees, recent immigrants, and undocumented populations, particularly low-income, 2SLGBTQI+, and racialized groups.
• Superior interpersonal, communication (written and verbal), problem-solving, organizational, time management, and self-care skills.
• Strong understanding and commitment to the principles and values of community health and community health centers.
• Ability to work collaboratively within an interdisciplinary health care team.
• Interest and commitment to working in a low-income, multilingual, multiracial community.
• Must be fully vaccinated against COVID-19.
• A criminal background check (Vulnerable sector) will be conducted.
