About the Role
Springbrook Wellness is expanding services at an Article 28 primary care clinic in Oneonta, NY, focusing on improving access to care for individuals with developmental disabilities. Responsibilities
- Provide primary care services to patients, with a focus on individuals with developmental disabilities.
- Complete patient assessments, diagnosis, treatment planning, follow-up care, and health education.
- Collaborate with clinical leadership, physicians, nursing staff, care teams, and community partners.
- Support the development of clinic processes, workflows, and patient care standards.
- Communicate clearly and compassionately with patients, families, caregivers, and support teams.
- Help create a positive, respectful, and accessible care experience for every patient.
