About the Role
The HOPE Program is seeking a Case Manager to provide counseling, guidance, referral, and case management services to a caseload of 120-150 individuals. This role supports participants and alumni in achieving their training, employment, and career advancement goals.
Responsibilities
- Provide comprehensive, participant-centered case management services to individuals experiencing barriers to employment and self-sufficiency.
- Maintain regular contact with participants through individual meetings, phone calls, virtual appointments, and classroom visits.
- Utilize trauma-informed, strength-based, and motivational interviewing techniques.
- Provide crisis intervention, conflict resolution, and supportive counseling.
- Connect participants to community resources and supportive services.
- Maintain current knowledge of community resources, New York City workforce systems, HRA, and community-based organizations.
- Contribute to HOPE’s working database of social service partners and resources.
- Maintain and cultivate partnerships for social service referrals.
- Collaborate with instructors, employment specialists, recruitment staff, and other program personnel.
- Facilitate workshops, orientations, and group discussions.
- Maintain accurate, timely, and confidential case notes, assessments, service plans, referrals, and participant records.
- Participate in participant conferences, staff meetings, and program planning discussions.
- Demonstrate professionalism, cultural humility, and ethical practice.
- Manage a diverse caseload while prioritizing multiple responsibilities and meeting program deadlines.
- Travel weekly between program sites and community partner locations, as required.
- Assist with intakes as needed and perform other duties as assigned.
Requirements
- Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or comparable educational background or training.
- Experience in case management or supportive services.
- Knowledge of trauma-informed care and New York City resources and services.
- Strong communication, organization, and time management skills.
- Experience facilitating workshops or group sessions.
- Proficiency in Google Workspace.
- Ability to travel between program sites.
- Professionalism, cultural humility, and a commitment to supporting diverse populations.

- $50,000.00 - $60,000.00 Salary/year
