About the Role

Provides peer support services and functions as a case manager responsible for the effective case management of an appropriate caseload under the direct supervision of a Qualified Health Professional.

Responsibilities

• Develop and maintain therapeutic relationships with residents.
• Assist in developing recovery and aftercare plans with residents.
• Help residents become aware of community social and support services.
• Model coping skills.
• Provide residents transportation to medical appointments.
• Engage residents in recreational activities, including walking, hiking, and sporting events.
• Work with residents to identify strengths for recovery efforts.
• Link residents to formal recovery supports.
• Provide travel training to residents regarding local transportation options.
• Actively participate in clinical supervision.
• Function as a supportive multi-disciplinary team member.
• Support resident retention initiatives.
• Facilitate didactic lectures on chemical dependency and related subjects.
• Facilitate and document group therapy sessions.
• Complete thorough admission procedures for residents.
• Complete accurate and thorough discharge summaries.
• Provide biweekly case management sessions for residents.
• Document monthly treatment plan reviews per compliance regulations, identifying treatment received and progress.
• Complete accurate and timely documentation of case management responsibilities.
• Demonstrate familiarity with and sensitivity to military culture.
• Display professional image, appearance, and behavior aligned with SJATRC Mission, policies, and procedures.
• Assure compliance with all necessary regulations.
• Actively participate in meetings, committees, and quality assurance activities.
• Maintain continued professional education.
• Maintain NYS Licenses and/or credentials as appropriate.
• Perform other duties as assigned by the Program Director.
• Conduct observational rounds of the facility throughout the shift.
• Provide conflict resolution support for and between residents.
• Provide support and counseling to residents outside of assigned caseload as needed.
• Respond to emergency situations in accordance with SJATRC policies and procedures.
• Communicate pertinent clinical information to other staff through documentation.
• Assess and monitor residents experiencing signs and symptoms of withdrawal using appropriate scales.

Requirements

• Experience working with diverse populations in a trauma-informed environment.
• Knowledge of outside community resources.
• Detail-oriented with professional communication and teamwork skills.
• Proficient in word processing/computer operations and telephone systems.
