About the Role
Peer Support services are peer-delivered services with a rehabilitation and recovery focus, designed to promote coping skills and facilitate the utilization of natural resources and recovery-oriented principles. This role uses trauma-informed, non-clinical assistance to achieve long-term recovery from a behavioral health disorder, assisting individuals and/or their families in initiating, maintaining, and enhancing the quality of their personal and family life in long-term recovery.
Responsibilities
- Provide advocacy, including assistance seeking and obtaining benefits and entitlements, food, shelter, and permanent housing.
- Assist recipients in participating in shared decision-making.
- Use Peer Bridgers to assist with linkages to and systems navigation within behavioral health and allied human services systems.
- Provide benefits advisement and planning.
- Assist in the development of psychiatric advance directives (PAD).
- Provide assistance advocating for self-directed services.
- Offer outreach and engagement services, including companionship and modeling of recovery lifestyle.
- Raise awareness of existing services and pathways to recovery, helping individuals remove barriers to access.
- Conduct interim visits with individuals and/or their families after discharge from Hospital Emergency Rooms, Detox Units, or Inpatient Psychiatric Units.
- Provide "Outreach" as a face-to-face service with a patient and/or their family to increase motivation for clinically indicated treatment.
- Assist in selecting and utilizing self-directed recovery tools such as Relapse Prevention Planning.
- Assist individuals in connecting to natural supports that enhance the quality and security of life.
- Connect individuals to “warm lines” and self-help groups in the community.
- Provide recovery education and counseling for individuals and their family members.
- Offer one-to-one peer support.
- Facilitate person-centered goal planning incorporating life areas such as community connectedness, physical wellness, spirituality, employment, and self-help.
- Assist with skills development to guide people towards a more independent life.
- Provide transitional supports, bridging individuals from jail, prison, institutions, or hospitals to their homes and the community.
- Arrange for appropriate services, discussing and making appointments with the individual/family prior to discharge.
- Provide pre-crisis and crisis support services, including companionship in emergency rooms or crisis units.
- Offer peer support in the home or community to support individuals before a crisis or relapse.
- Develop crisis diversion plans or relapse prevention plans.
Requirements
- High school diploma or equivalent.
- "Lived" / relevant experience.
- Clean driving record for 3 years.
- Must maintain certification requirements.
