About the Role

Help people experiencing homelessness find a place to call home and become self-sufficient by identifying causes of homelessness, coordinating service teams, developing independent living plans, and providing referrals for employment, education, medical, mental health, childcare, and substance abuse services.

Responsibilities

  • Identify the causes of client homelessness.
  • Coordinate with service team members to develop independent living plans.
  • Meet regularly with clients to help them work their plan and find a home.
  • Provide direction and referrals for employment, education, medical and mental health services, childcare, substance abuse, and more, depending on client needs.

Requirements

  • Ability to keep track of multiple priorities.
  • Ability to complete tasks on time.
  • Ability to keep appropriate people informed of status.

Education Level

  • Bachelor's degree in social work or a related field OR equivalent experience and skills.

Salary/Compensations

  • Actual compensation will not be less than the applicable minimum wage, prevailing wage, or exempt salary requirements under federal, state and local laws.
