About the Role
Villa of Hope helps youth and families rebuild relationships, recover from trauma and renew Hope for their future. This role provides regular care management services to an assigned caseload, facilitating the development of a Care Team and an individualized, strength-based Plan of Care.
Responsibilities
- Exercises full compliance with the Agency’s Code of Conduct, all Agency Policies and Procedures.
- Maintains confidentiality and security for all client and staff related materials and/or records.
- Practices adherence to Program/Agency standards and ensures documentation/policy requirements are followed.
- Demonstrates, promotes, and practices cultural competency towards clients through respect and understanding.
- Recognizes and embraces the diversity of teams, supports and develops the strengths of each individual adult and their family.
- Maintains positive relationships with clients/families and other service providers in the community.
- Provides regular care management services to assigned caseload, including face-to-face visits.
- Maintains the required amount of billable services for each adult on their caseload.
- Facilitates the development of a Care Team and carries out an individualized and strength-based Plan of Care.
- Assists clients in managing and planning for safety needs and creating individualized safety/support plans.
- Makes independent decisions and recommendations regarding immediate risk factors presented in home-based sessions.
- Assures clients and families obtain necessary services for physical and mental health and safe community living.
- Provides linkage/referrals to community resources for mental health, social, physical, and housing/basic needs.
- Informs and educates clients and their families around a psychiatric diagnosis and its impact on home and community.
- Arranges own schedule to be available to clients and families in their homes/community, including working evenings if needed.
- Completes assessments of all clients, including HARP if applicable, with a comprehensive view of strengths, challenges, skills, and supports.
- Documents all client contacts and maintains clinical records in accordance with Agency Standards.
- Ensures documentation is accurate and completed on time.
- Participates in regular weekly supervision.
- Remains current in the field and supports best practice models within the program.
- Participates in ongoing training and professional development provided by the agency.
Requirements
- A Bachelor’s of Arts or Science are preferred.
- An Associate’s Degree with 1 year relevant experience in the Human Service, Health Care, or Mental Health fields.
- A strong knowledge of Adult Mental Health preferred.
- Valid NYS driver’s license in good standing and has use of an automobile.
- Flexibility to meet with clients during evening hours if needed.
- Ability to drive.
- Ability to walk short distances.
- Ability to use stairs.
- Ability to leave a location quickly.
