About the Role
This role involves supporting safe and timely hospital discharges, ensuring individuals have the necessary support to regain independence. You will work collaboratively within a busy hospital adult social care team to assess needs, coordinate care, and make a tangible difference in people's lives during critical times.
Responsibilities
- Assess needs, coordinate care, and help people regain independence after a hospital stay.
- Collaborate with families, carers, NHS colleagues, and other professionals to ensure safe hospital discharges.
- Conduct strengths-based assessments under the Care Act 2014.
- Support best interest decisions.
- Create personalized support plans to reduce the risk of readmission.
- Engage in safeguarding work to protect residents with diverse needs.
- Promote reablement where appropriate.
- Support unpaid carers through assessments, information, and referrals.
- Maintain accurate and timely case records.
- Contribute to a positive team culture.
- Coordinate complex discharges.
- Problem-solve with partners.
- Make quick decisions that have a lasting impact.
Requirements
- Qualified social worker with current Social Work England registration.
- At least one year of post-qualifying experience.
- Strong working knowledge of the Care Act 2014.
- Strong working knowledge of safeguarding processes.
- Strong working knowledge of hospital discharge protocols.
- Highly organised and confident working under pressure.
- Excellent communication skills.
- Ability to build strong working relationships.
- Confident writing clear, concise assessments and reports.
- Comfortable using electronic case management systems.
- Ability to stay calm in a fast-paced setting.
- Ability to make sound decisions.
- Commitment to keeping the person at the centre of all actions.
