About the Role

Under the direction of the Nurse Manager, responsible for ensuring the proper delivery of nursing services in compliance with OPWDD and DOH regulation. Practices and promotes the agency’s mission statement in every interaction.

Responsibilities

• Practices and promotes Citizens mission statement in every interaction.
• Supports people to live the life they choose through family and community engagement.
• Provides hands-on clinical support including assessments, medication administration, emergency response, and nursing skills within scope of practice.
• Provides clinical direction to LPN & AMAP staff and ensures nursing skills are performed to standards.
• Ensures the implementation of primary nursing principles in accordance with OPWDD and DOH regulation, meeting health needs in assigned homes.
• Provides on-call coverage on a rotating 24/7 basis for medical concern triage.
• Completes a plan of nursing services (PONS) for each person supported, ensuring thorough and up-to-date documentation (CFA, PONS, Braden scale).
• Initiates and completes training upon changes in a person's status and at least annually.
• Ensures completion of all ordered consultations, lab work, medical visits, and medications for a designated caseload.
• Participates in nursing staff meetings and attends team, administrative, and management meetings to communicate medical information and seek recommendations.
• Ensures nursing department adherence to all ICF regulations and participates in OPWDD surveys.
• Confers with individuals receiving supports and services and their family members/correspondents to discuss medical concerns.
• Ensures work-related safety procedures are in place and adhered to by staff regarding workplace safety and infection control.
• Reports reasonable cause to suspect abuse or neglect or a potentially hazardous work environment to the Justice Center as a mandated reporter.
• Follows all agency incident policy and procedures.
• Adheres to all regulations, agency policy and procedures, the employee handbook, and agency and program training requirements.
• Performs other duties as instructed by supervisor.

Requirements

• Requires regular and substantial unsupervised or unrestricted physical contact with people receiving services.
• Subject to successful completion of fingerprint/criminal background check.

Skills

• NYS Registered Nurse License

Experience Level

• 1 year experience preferred but not required
