About the Role

Seeking compassionate individuals looking to make a difference in someone’s life and work for an organization that appreciates and recognizes employee success. This role provides outreach and enrollment services to individuals eligible for New York State’s Health Home program, gathering assessments, confirming eligibility, and identifying supportive service needs. The Care Manager works with individuals and providers to create a comprehensive Plan of Health Care, coordinate services, and document progress.

Responsibilities

  • Assess, engage, and enroll clients into the health home program per NYSDOH and Lead Health Home regulations.
  • Complete home visits with enrolled members at least once per month, with frequency determined by client need.
  • Collaboratively complete a yearly assessment of needs, strengths, and goals with the member/family and care teams.
  • Facilitate a yearly meeting with the member's care teams and work within that team to develop a comprehensive Plan of Care.
  • Provide core services including care coordination, comprehensive care management, transitional care, crisis intervention, and caregiver/family support.
  • Document all case activities in an Electronic Health Record, adhering to NYS guidelines for documentation quality and timeframes.
  • Ensure monthly billing standards are met for each enrolled member and submit a billing questionnaire within the month of activity.
  • Participate in mandatory and optional training.
  • Participate in weekly supervision.

Requirements

  • Bachelor’s Degree with two years of experience; Master’s Degree with one year of experience preferred.
  • Relevant experience in serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services.
  • Experience coordinating and participating in team settings.
  • Solid writing and verbal communication skills.
  • Ability to work effectively and knowledgeably across a broad spectrum of cultural, ethnic, and racial communities.
  • Ability to work in an effective and focused manner when self-directed and beyond immediate oversight.
  • Ability to deliver family-driven, youth-guided services.
  • Computer proficiency including word processing skills.
  • Ability to provide culturally competent practice.
  • Valid driver’s license that meets The Salvation Army insurance requirements.
  • Access to a vehicle to provide home visits.
