About the Role

Health Quest Home Care is seeking dedicated individuals to join their home healthcare team. This role involves independent work at the top of professional licenses while collaborating within a coordinated home health and Nuvance Health continuum of care team. The position offers the opportunity to provide care in patients' homes, working closely with physicians and families.

Responsibilities

• Assess physical, emotional, socio-economic, and environmental needs of patients and families.
• Provide physician-ordered services as per the plan of care, communicating with involved physicians and healthcare practitioners.
• Develop, implement, and evaluate the plan of care in partnership with the patient, caregiver, representative, and physician(s).
• Coordinate services with the patient, family, and others involved in the care plan, providing counseling and education.
• Regularly evaluate patient progress, take prompt action on changes in condition, and terminate care when goals are met or as required by agency policy.
• Participate in the agency's quality assurance, performance improvement program, and in-service training.
• Identify and refer patient and family needs for other home health services when appropriate.
• Supervise homemaker/home health aide services according to State, Federal, and Joint Commission guidelines, submitting periodic evaluations.
• Fulfill compliance responsibilities, including preparing clinical notes and attending in-service training.

Requirements

• RN diploma with over 3 years of job-related experience considered in lieu of an Associate's Degree.
• Current RN License in the State of New York.
• Strong research skills.
• Clear and concise interpersonal communication skills.
• Knowledge of systems integration.
• Minimum of one year of experience.
• Experienced in a qualified home health agency (Desired).
