About the Role

Achieve Behavioral Health is launching a new 68-capacity Adult Assertive Community Treatment (ACT) program in Rockland County and seeks an experienced clinician and leader to build it from the ground up. This role involves creating the program, building a team, establishing culture and workflows, and serving high-need adults, with the support of an established organization.

Responsibilities

  • Provide clinical leadership and supervision to a multidisciplinary ACT team
  • Participate in recruiting and developing the new team
  • Provide direct clinical services to ACT participants
  • Facilitate the team's daily morning meeting
  • Provide individual and field-based supervision
  • Maintain fidelity to the NYS OMH ACT model
  • Develop relationships with hospitals, SPOA, AOT, and other community partners
  • Support the team's gradual development toward full census
  • Coordinate care for individuals receiving court-ordered AOT services
  • Monitor documentation, clinical quality, CAIRS reporting, and chart compliance
  • Use program data and PSYCKES to identify opportunities for quality improvement
  • Participate in the team's rotating on-call schedule
  • Help establish a strong, collaborative, recovery-oriented team culture

Requirements

  • Master's degree with current NYS licensure such as LCSW, LMSW, LMHC, etc.; OR NYS-licensed RN; OR licensed physician
  • At least 5 years of experience working with adults with serious mental illness
  • At least 2 years of supervisory experience
  • Experience working on a multidisciplinary or community-based behavioral health team
  • Experience with evidence-based approaches such as motivational interviewing, harm reduction, or trauma-informed care
  • Valid driver's license
  • Prior ACT experience is strongly preferred, particularly experience as an ACT Team Leader, Assistant Team Leader, or senior ACT clinician
  • Experience as a clinical supervisor or senior clinician in AOT, mobile crisis, intensive care management, community-based mental health, or other intensive behavioral health programs is also considered
