About the Role
The Senior Manager is responsible for overseeing the Health Home Census and individual Case Managers, promoting effective education, self-management support, and timely healthcare delivery to achieve optimal quality and financial outcomes. The role emphasizes professional conduct and the promotion of respect, honesty, care, and dignity for patients and the healthcare team, with a commitment to improving community health.
Responsibilities
- Promotes a safe, cooperative, and professional healthcare environment.
- Adheres to all hospital policies, procedures, rules, and regulations.
- Prioritizes customer service, treating all individuals professionally and courteously.
- Effectively communicates SBH services and ensures concerns reach the appropriate personnel.
- Demonstrates standards of performance and behaviors consistent with DRIVE to Patient-Centered Excellence.
- Serves as a liaison between Health Home staff, SBH Behavioral Health and Medical providers, patients, and management to improve service coordination and communication.
- Collaborates with SBH Behavioral Health to identify appropriate patients for care management and supervises the referral process.
- Provides direction, support, and supervision to employees, holding staff meetings to discuss issues.
- Monitors employee worklists for timely documentation and compliance with BAHN and NYS DOH Health Home regulations.
- Reviews and approves documentation, including consents, assessments, care plans, contacts, and referral management.
- Ensures coordination and linkage to behavioral health, medical, and support services.
- Acts as a conduit between Health Home, Behavioral, and Medical providers to ensure client needs are met.
- Provides crisis management coordination and ensures timely completion of incident documentation.
- Responsible for training new employees.
- Meets with patients, providers, and staff to develop and coordinate treatment plans, and meets with family members as needed.
- Participates in the Health Home Management Team and executes initiatives.
- Manages and enters E-time, vacation, and elective days.
- Manages Healthfirst members’ Gaps in Care.
- Reviews charts in BAHN and EPIC for program compliance.
- Receives, reviews, processes referrals, and creates MAPP Segments.
- Screens internal cases for Health Home Plus.
- Attends and participates in scheduled department and division meetings.
- Performs other duties as assigned by Leadership.
Requirements
- Master’s degree in social work or another human services field.
- Two years of management experience in a behavioral health setting.
- Three years in care management with progressive responsibility.
- Ability to interact with all levels of staff in a professional manner.
- Proficiency in Microsoft Office, Excel, and EMR.
- Strong written and verbal communication skills.
- Bilingual-Spanish/English preferred.
