About the Role

The Self Direction Coordinator (SDC) manages individual financial accounts based on approved Self Direction Plans/Budgets. The SDC educates the support circle on regulations and policies, explains fiscal procedures, and maintains essential participant documents. This role serves as the primary contact for fiscal intermediary services.

Responsibilities

  • Manage individual financial accounts based on approved Self Direction Plans/Budgets.
  • Participate in all Life Plan meetings and educate the circle of support about current regulations and specific policy and procedure.
  • Explain all fiscal procedures and required waiver documentation to the participant.
  • Provide participants with copies of all written policies and procedures.
  • Maintain current documents for each Self Direction participant, including ISP/LP, Self Direction Plan, Staff Action Plan, budget attachments, and Monthly Summary Notes.
  • Maintain all current billing documents for each Self Direction participant.
  • Ensure all payments are correctly processed in a timely manner.
  • Rectify and process expenses needing more information.
  • Serve as the main contact point for all questions and concerns for anyone supported by AIM for fiscal intermediary services.
  • Participate as an active member of the individuals’ program team and Circle of Support.
  • Maintain schedule flexibility to ensure individual care and program implementation.
  • Perform any other related duties as requested by supervisor.

Requirements

  • Associate’s degree in a Human Services Related Field with 2 years’ experience working with individuals with disabilities preferred.
  • High School Degree with 4 years’ experience working with individuals with disabilities preferred.
  • Program specific training (provided internally).
  • Valid NYS Driver’s License acceptable to Agency Insurance standards.
  • Maintain valid insurance and provide insurance card if using your own vehicle.
  • Ability to exhibit/maintain a professional, kind, courteous and patient manner at all times.
  • Proficient (spoken and written) in one or more languages other than English (Spanish, Mandarin, Russian, and/or Yiddish preferred, but not exclusive).
  • Must be able to lift a minimum of 50 pounds.
  • Ability to perform bending/reaching/pushing/pulling.
  • Effective verbal and written communication skills.
  • Reading and Math skills at a level to perform job duties.
  • Demonstrated interest and ability to make possible the personal outcomes of individuals receiving services.
  • Adherence to Agency policies and procedures.

Skills

  • Proficient (spoken and written) in one or more languages other than English (Spanish, Mandarin, Russian, and/or Yiddish preferred, but not exclusive).
  • Effective verbal and written communication skills.
  • Reading and Math skills
