About the Role
Case Managers in the Permanent Supportive Housing (PSH) Department serve residents at a designated housing site, providing individualized case management and support services to formerly homeless adults. Services include clinical needs assessment, service planning, supportive counseling, referrals, and facilitating groups.
Responsibilities
- Perform intake assessment of assigned residents, including health status, psychosocial history, financial status, substance use history, and housing history.
- Conduct regular outreach to residents and respond to incidents or documentation indicating a resident may be in crisis or jeopardizing their housing.
- Develop and implement individualized Service Plans based on resident goals and needs.
- Provide referrals to internal or external resources for substance use, vocational training, legal services, immigration services, health services, and housing.
- Assess resident eligibility for benefits and assist with applications.
- Assist residents with budgeting and money management plans.
- Advocate on behalf of residents with Property Management regarding issues that may jeopardize housing.
- Assist with conflict resolution to ensure a comfortable living environment.
- Participate in Wellbeing Checks and Emergency Safety Checks.
- Develop and facilitate at least one weekly therapeutic or psycho-educational group.
- Assist with organizing and carrying out program events and community-building activities.
- Maintain up-to-date, complete, confidential, and professional documentation in resident files.
- Complete all necessary paperwork and administrative requirements within the required timeframe.
- Assist residents with planned exits from the program, including discharge planning and referrals.
- Assist in developing and implementing new programs, groups, and activities.
- Provide back-up coverage for other case managers.
- Share information and new resources with residents and staff.
- Follow and implement all applicable policies and procedures.
- Establish and maintain positive working relationships with employees, volunteers, residents, and outside agencies.
- Attend all assigned meetings.
- Perform other duties as assigned.
Requirements
- MA/MS + at least 2 years’ experience, or BA/BS + at least 4 years’ experience, or CHW Certificate/CADC + 4 years’ experience.
- Knowledge of local social services and community resources for underserved populations.
- Patience, compassion, and empathy towards participants.
- Excellent charting, recordkeeping, and professional writing skills.
- Ability to quickly assess and respond to various situations, including emergencies and crises.
- Computer skills and moderate experience using Microsoft Outlook, Word, and other MS Office applications.
- Strong interpersonal skills, a team approach, and effective oral and written communication.
- Demonstrated cultural competency in working with diverse, underserved populations.
- Ability to maintain professional, respectful, honest, and calm interactions.
- Ability to work as part of a multi-disciplinary team.
- Comfortable facilitating groups and hosting events and activities.
- Commitment to the mission and values of Episcopal Community Services.
- Must secure finger image screening, annual TB screening, and be fully vaccinated showing proof of COVID vaccination and boosters.
- Able to meet physical requirements including long periods at a desk/computer, some travel, and lifting up to 20 lbs.
