About the Role
The Case Planner in the TFBH program provides casework, group work, and advocacy services to children and families, focusing on enabling optimal bio-psychosocial development through supportive work with foster parents and rehabilitative work with the child’s family. This role involves handling a caseload, gathering information, providing services to meet child needs, focusing on permanency planning, and interacting with governmental and community agencies.
Responsibilities
- Obtains data regarding child’s and family’s social and medical history.
- Provides case planning.
- Advocates for and obtains appropriate services for the child, including educational, clinical, and recreational services.
- Meets regularly with foster parents to clarify issues, provide and/or obtain concrete and supportive services.
- Monitors the quality of care provided to the child.
- Keeps foster parents advised of child’s condition and prognosis.
- Engages the child’s family in casework and group work activities aimed at sustaining relationships through visiting.
- Works towards family reunification if possible.
- Assists with adoption related activities if necessary.
- Arranges for regular training of foster parents.
- Shares 24-hour 7-day emergency cell phone coverage with the Behavior Specialist.
- Responds to emergencies after hours or on weekends.
- Performs related duties as required.
Requirements
- MSW; M.A. + 1 year experience; B.A. + 3 years related experience; or B.A. + 5 years unrelated experience.
