About the Role
The Case Manager provides and documents clinical and case management services for assigned clients. The role requires an active listener who provides high-quality customer service and exhibits cross-cultural awareness, promoting an equitable and inclusive anti-racist organizational culture.
Responsibilities
- Provides comprehensive case management services for assigned clients.
- Offers support and resources/referrals for clients to meet objectives, including entitlements, housing, employment/education/vocational training, health, mental health treatment, substance abuse services, and social/community integration.
- Provides counseling and support to clients on an ongoing basis, including crisis intervention, training and support on daily living skills, assistance in problem-solving, conflict resolution, and advocacy.
- Assesses client needs and develops service plans or referrals to achieve client self-sufficiency.
- Conducts fieldwork, including home visits, escorting clients to appointments, and outreach.
- Facilitates client groups.
- Monitors client medications as required.
- Conducts client intake and updates information in client database systems.
- Completes and submits incident reports and related notices within 24 hours.
- Maintains and submits all necessary documentation and reports accurately and timely.
- Enters progress notes after each client meeting within 48 hours or as mandated.
- Completes Intake/Admission documentation within 24 hours of client admission as mandated.
- Documents assessments in client records within the required timeframe.
- Updates information in client records as required.
- Documents incidents and distributes related notices within 24 hours when the Case Manager has witnessed the incident.
- Submits all required paperwork for supervisor approval within 48 hours.
- Files paperwork in client charts within 48 hours after approval/signing by supervisor.
- Communicates verbally and in writing in a professional manner.
- Attends staff meetings, trainings, and supervisory sessions as required.
- Responds to all communications in a timely manner.
- Performs all other duties as assigned and required.
Requirements
- Bachelor’s degree required.
- Bachelor’s degree in Social Work or other related field preferred.
- Minimum one year of case management, individual or group counseling experience preferred.
- Knowledge of NYC housing subsidy programs.
- Experience with people experiencing homelessness, mental illness, and/or substance abuse population preferred.
- Required to work a flexible schedule including weekends, holidays, and evenings.
- Comfortable with using public transportation to travel to and from various program sites.
- Valid driver’s license with a clean driving record a plus.
Skills
- Proficiency in Microsoft Office (Word, Excel, Outlook, etc.)
- Proficiency in electronic record-keeping systems (Awards, Cares, Caires, etc.)
- Ability to maintain accurate and up-to-date case notes, documentation, and records.
- Excellent organizational and time management skills.
- Excellent communication, interpersonal abilities, and follow-up skills.
- Ability to work effectively with individuals from diverse backgrounds and cultures.
- Foreign languages a plus.
