About the Role

This is a per-visit role perfect for supplemental income, not primary employment, focused on establishing and growing Haven Home Care's presence in Eastern Ohio. The Licensed Practical Nurse will complete open visits, conduct supervisory visits for Home Health Aides, and perform pre-employment testing for aide candidates.

Responsibilities

• Assist in carrying out personalized care plans as directed.
• Prepare accurate and thorough documentation according to agency standards and regulatory requirements.
• Conduct regular supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients.
• Conduct skills tests and written tests for Home Health Aide candidates.
• Foster cooperative relationships among the healthcare team by understanding each person's role and actively participating in case conferences.
• Assume responsibility for ongoing professional growth through participation in nursing organizations and in-service programs.

Requirements

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver's license, reliable transportation, and current auto insurance.
• Current CPR certification.
• Successful completion of criminal background check, skills test, and written nursing test.
• Residence in Trumbull or Mahoning County preferred.

• Communication skills with ability to interact professionally with clients, families, and healthcare team.
• Knowledge of home health regulations and practices.
• Ability to work independently and schedule visits efficiently.
• Proficiency in utilizing calculator, multi-line telephone.
• Competent operation of company-provided iPad with data plan.
• Appropriate use of company-provided name badge and cardigan with "Haven" logo.

Experience Level

• One or more years in a home health agency or in a hospital setting. Home health experience preferred.

Salary/Compensations

• $25 per supervisory visit
• $35 per open visit
• $35 per skills test
• $35 per hour of office time for any/all written tests
• Mileage reimbursement Paid at $0.655 per mile
