About the Role
CBI Home Health supports people at home and in the community through services such as our Integrated Care Programs, designed to support individuals with complex care needs transitioning from hospital to home. Our teams focus on helping clients restore health and function through a restorative care approach, enabling them to regain independence and thrive in their communities. These programs are person-centered, team-based, and prioritize communication, coordination, and the individual's best interests.
Responsibilities
- Provide OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
- Use a collaborative, interdisciplinary approach to care and services.
- Support a restorative approach to health, offering interventions, health coaching, and self-management support.
- Utilize digital platforms for care delivery, collaboration, documentation, and intervention.
Requirements
- Registered member in good standing with the College of Occupational Therapists of Ontario.
- Ability to travel within the community.
- Valid driver's license and insurance required.
- Embrace person- and family-centered care with passion and empathy.
- Exercise good judgment and critical thinking in client care decisions.
- Excellent communication skills.
- Thrive in both team settings and working independently.
- Value the role of a therapist in enabling clients to remain at home when it aligns with their personal goals.
