About the Role
Incumbent is responsible for providing professional nursing care and performing case management duties for patients of the Seneca Nation Health System (SNHS) in accordance with established standards and practices, within the bounds of training and professional licensure.
Responsibilities
- Identifies patient problems, goals, and interventions to meet individual needs and healthcare goals.
- Assesses patient progress, identifies barriers, and collaborates with patients and providers to find solutions.
- Assesses patient support systems, caregiver resources, and community benefits, making referrals as appropriate.
- Develops and maintains team-oriented relationships with healthcare staff.
- Evaluates and reports on patient outcomes with interdisciplinary teams.
- Provides direct patient care and assists with examinations and procedures.
- Assists patients with treatment adherence and medication compliance, monitoring status changes.
- Provides culturally sensitive care for patients of all ages.
- Handles patient care communications effectively.
- Orients, trains, and instructs assigned personnel.
- Accurately records patient care information and ensures timely data entry into the electronic health record.
- Maintains a safe, comfortable, and therapeutic environment for patients and families.
- Promotes a cooperative and cohesive workplace environment.
- Participates in and promotes Patient Centered Medical Home (PCMH) guidelines and procedures.
- Performs electronic health record functions and duties required for patient care.
- Documents all patient contacts according to the Privacy Act of 1974.
- Enhances professional growth through educational programs and literature review.
- Maintains a clean and sanitary workplace through general maintenance, housekeeping, and cleaning.
- Assists with monitoring adherence to departmental processes.
- Ensures confidentiality of records according to the Privacy Act of 1974 and HIPAA.
- Upholds and promotes the mission statement and values of the Seneca Nation Health System.
- Provides care through a trauma-informed lens, incorporating principles of safety, trustworthiness, collaboration, empowerment, and cultural sensitivity.
- Attends all mandatory staff meetings, trainings, and in-services.
- Follows all department, SNHS, and Seneca Nation policies.
Requirements
- Must possess and maintain current NYS licensure as Registered Professional Nurse.
- Minimum of 3 years’ clinical experience providing direct patient care as an RN.
- Basic Life Support (CPR and AED) certification, or the ability to obtain certification within six months of hire.
- Responsible to maintain up-to-date certification.
