About the Role
The Social Worker I will work autonomously to build 1:1 relationships with residents, providing a comprehensive range of social work services to enable them to establish and maintain safe, independent lifestyles. WSFSSH supports MSW-level social workers in obtaining their licensing through study groups, exam payment, and licensing fee coverage.
Responsibilities
- Assist residents with obtaining and maintaining medical and mental health services, entitlements, and home care.
- Assist residents with establishing and/or maintaining family and social relationships.
- Assist residents with budgeting and income maintenance to ensure on-time rent payment.
- Assist residents with money management.
- Provide crisis intervention services as needed.
- Provide client appointment escorts as needed.
- Provide individual counseling.
- Assist residents in participating in socialization activities through in-house and community referrals.
- Follow up with residents and other service providers during out-of-facility stays (e.g., hospitalizations, short-term rehabilitation).
- Oversee the discharge planning process for residents, including completing necessary forms and applications.
- Provide group work services for residents (recreational, educational, and/or clinical).
- Participate in the intake process of prospective residents.
- Share resident progress, needs, and problems with other staff, adhering to the WSFSSH Confidentiality Policy.
- Complete non-reportable and reportable incident reports.
- Maintain competency in required software and databases.
- Implement emergency procedures as necessary.
- Attend supervision, team, and staff meetings.
- Maintain thorough case records, including case notes, support plans, and psychosocial histories within required timeframes.
- Perform other duties as assigned.
Requirements
- Good written and oral communication skills.
- Good organizational ability, time management, and interpersonal skills.
- Desire or interest in working with and supporting an aging population.
- Fluency in written and oral English required; bilingual is a plus.
- Experience with older adults and people with mental illness and/or substance use preferred.
- Must be able to stand for extended periods of time.
