About the Role
Coordinate appropriate medical care, dental care, and mental health services to adult clients enrolled in the Assertive Community Treatment (ACT) program. This role involves a community-based approach, supporting the RN and Psychiatric Nurse Practitioner on the ACT team. You will meet with clients to assist with medication administration and other needs.
Responsibilities
- Review lab and test results, reporting critical values to RN and providers
- Medication management including ordering medications and obtaining prior authorizations
- Dispense medications; picking up medications, creating medication boxes, overseeing client medication administration
- Communicate with Director and ACT team regarding observation in client’s health/behavior
- Provide client and family education regarding medications and treatments
- Meet with clients and families within their homes and communities within Saratoga, Warren and Washington counties
- Collaborate with the ACT Team during daily team meetings regarding concerns and behaviors
- Nursing documentation using the electronic health record
- Support all admissions and discharges of clients and their medication needs
- Work in collaboration with service providers and pharmacies to support client needs
- Other duties as assigned within scope of practice and based on agency needs
Requirements
- LPN Licensed in New York State
- CPR Certification required
- 1 to 2 years of experience
- NYS License required as travel is required
