About the Role
The Psychiatric Nurse Practitioner provides psychiatric medication services to patients, many of whom have co-occurring mental health and substance use concerns. This role collaborates with a multidisciplinary mental health team, focusing on a patient-centered, harm reduction, and trauma-informed approach to care.
Responsibilities
- Screens patients for common psychiatric, substance use, and/or medical co-morbidities and evaluates co-existing conditions and their implications on patient care.
- Ensures timely completion of admission assessments, psychiatric assessments, histories, follow-ups, and lab testing as required.
- Prescribes and titrates appropriate medication dosages.
- Writes medical orders based on individual patient needs.
- Works within an integrated treatment model, coordinating care with internal and external providers.
- Serves as a role model for positive and cooperative relationships with staff and clients.
- Adheres to OMH regulations and Evergreen Health policies, including clinical documentation using an electronic medical record system.
- Collaborates with and supports team members to ensure a patient-focused approach to care.
- Attends and participates in case conferences, center meetings, and program initiatives.
Requirements
- Ability to travel between Evergreen locations is required.
- Awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential.

Experience Level
- Two years of direct care practice experience as a PNP preferred
Education Level
- Graduate of an accredited Psychiatric Nurse Practitioner training program
- Certified Psychiatric Nurse Practitioner
- Licensed to practice in New York State
- Current BLS/CPR certifications
Salary/Compensations
- Salary range posted; actual rate based on skills, experience, education, internal equity, and market considerations.
