Responsibilities

  • Promotes a safe, cooperative, and professional healthcare environment to ensure optimum patient care.
  • Adheres to all hospital policies, procedures, rules, and regulations.
  • Prioritizes customer service, treating patients, visitors, and coworkers professionally with courtesy and tact.
  • Effectively communicates about hospital services and ensures concerns reach the appropriate personnel.
  • Interviews patients and/or families to obtain psychosocial information and assess functioning related to safety and discharge planning.
  • Formulates an assessment and treatment/discharge plan highlighting a clinically realistic and sensitive understanding of patient needs.
  • Participates in scheduled multidisciplinary rounds and meetings.
  • Provides direct social work/discharge planning services, including community resource advocacy and maintaining rapport with community agencies.
  • Documents all work activity in the patient’s medical chart and Allscripts according to departmental and regulatory guidelines.
  • Engages with patients and/or family members with respect to cultural differences and individuality.
  • Demonstrates consistent attention to patient confidentiality.
  • Counsels patients, families, and significant others regarding lifestyle changes and functional coping patterns.
  • Completes and submits monthly statistics in a timely manner.
  • Attends all scheduled Social Work/Discharge Planning Department meetings.
  • Speaks to patients, families, and significant others with age-appropriate conversation and demeanor.
  • Provides social work rotating weekend coverage in Pediatric Emergency Department, Nursery, Labor and Delivery, and other areas as necessary.
  • Attends scheduled meetings, participates in White Board Rounds, discharge planning meetings, and patient conferences, providing supporting documentation.
  • Attends scheduled supervision and obtains consultation when needed.
  • Performs other duties as assigned.

Requirements

  • Must possess a Master’s Degree in Social Work from an accredited school of social work.
  • Must possess New York State Certification (LMSW) at time of hire or obtain within 6 months of start date.
  • Requires two (2) years of experience in a hospital setting related to the department to which the worker will be assigned.
