About the Role

Responsibilities
- Perform initial and ongoing comprehensive assessments of the impact of terminal disease on patients' physical, psychosocial, and environmental needs and ADLs.
- Implement individualized POC and recommend revisions as necessary.
- Collaborate care amongst patient/caregiver, physicians, and healthcare professionals.
- Attend Inter-Disciplinary Group (IDG) meetings and participate in patient care planning.
- Perform on-call responsibilities and provide on-call service to patients/families.
- Ensure timely submission of all documentation.
- Perform other duties as required.
- Ensure compliance with local, state, and federal laws, Medicare regulations, and personnel policies.
Requirements
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State Registered Nurse License
- Current Driver's License
- Reliable transportation
- Ongoing valid and current auto liability insurance
- Two years of experience as an RN preferred in a healthcare/clinical setting
