About the Role

The Community Wellness Specialist II provides emotional support and related service offerings via outreach and wellness activities. This role involves screening, referral, and linkage to community-based resources using active supportive listening.

Responsibilities

  • Demonstrates agency core values in day-to-day activities.
  • Works with individuals, families, and groups to provide outreach, emotional support, individual and group sessions, public education, and referrals.
  • Represents the program in the community and networks with other agencies and partners.
  • Provides presentations to community groups on emotional reactions, coping skills, and stress management.
  • Advocates on behalf of the client to ensure needed services are obtained.
  • Provides service referral, linkage, and related activities.
  • Utilizes Motivational Interviewing techniques to increase likelihood of engagement.
  • Applies knowledge of Solution Focused Therapy.
  • Utilizes supportive, active listening skills.
  • Conducts brief educational or supportive contact.
  • Facilitates support and educational groups.
  • Assists with self-help groups.
  • Performs screening, referral, and resource linkage.
  • Engages in community support and networking.
  • Conducts public education.
  • Develops and distributes educational materials.
  • Drives a personal vehicle for client meetings and counseling activities.
  • Performs other duties as assigned.

Requirements

  • Must be proficient in the use of computers and computer software.
  • Must be familiar with all Microsoft products.
  • Must possess a valid Driver’s License with a satisfactory driving record.
  • Must possess a personal vehicle for job requirements.

Experience Level

  • One to three years of experience in a Physical Health, Behavioral Health or Human Services setting is preferred.
  • One to three years in Mental Health or Human Services setting dependent on certification/education is preferred.

Salary/Compensations

  • $22.50-$25.60/hour
