About the Role
Visiting Nurse Service Westchester, established in 1901, is seeking a Physical Therapy Assistant for Home Care. This role is available in Westchester, Rockland, Putnam, or Dutchess counties. Responsibilities
- Provides direct care to treat the medical/surgical condition through the use of accepted physical therapy modalities as assigned by the physical therapist.
- Instructs the patient/family and home health aide in the home exercise program and other aspects of patient care as directed by the physical therapist and documents the response to instruction.
- Provides written home exercise program to patient as needed and also provides a copy for the patient’s record.
- Continually monitors the patient's condition and promptly reports any changes to the physical therapist and clinical manager.
- Documents all clinical visits and patient's response to therapy interventions in the EMR.
- Submits completed clinical documentation in accordance with agency policies.
- Participates in patient care conferences.
Requirements
- NYS Professional License and current registration to practice as a Physical Therapy Assistant.
- Minimum of two (2) years direct clinical experience.
- Home care experience preferred.
- Automobile in good working order and auto insurance in the amount specified by VNSW.
- Valid driver’s license.
- Knowledge of HCHB Clinical Software preferred.
- Must maintain CEU requirements.
- Professional Liability Insurance in the amount specified by VNSW.
