About the Role
Ensure the safety and well-being of all participants in a community-based program. Act as a teacher, role model, and support to individuals, treating everyone with dignity and respect. Provide a caring, enabling, empowering, and supportive setting that promotes increased independence.
Responsibilities
- Assist, supervise, support, and instruct individuals to promote independence and community integration.
- Teach skills in life safety, communication, cooking, budgeting, shopping, money management, and socialization.
- Facilitate recreational activities, both new and preferred.
- Engage individuals in group and individual social and health-related activities.
- Assist in the development and implementation of habilitation plans and goal development.
- Complete daily documentation accurately and timely, including Range of Scan, supervision, and trip forms.
- Provide safe transportation to and from the day program and during community outings.
- Collaborate effectively as a team member with supervisors and co-workers.
- Follow management instructions and maintain accountability for whereabouts.
- Maintain a safe, clean, and stress-free environment.
- Complete all required trainings.
- Perform other duties as assigned to ensure the operation of the day program.
